To request the installation, removal, or relocation of a vending machine,
please complete the form below, then click submit request. All vending
placement requests must be reviewed and approved by Procurement and Business Services; the Department of Environmental Safety, Sustainability & Risk; and the applicable
vendor. Following review, vending placement requestors will be informed
of placement decisions and, if applicable, informed of the scheduled
date for installation, removal, or relocation.
